Injury settlement doesn't pay medical bills?

The subject was hit by a vehicle while riding a bicycle in Winston-Salem, NC. He obtained an attorney. Almost a year from the date of the accident, he was contacted by the attorney and told that his settlement is $9,250.
The subject's medical bills are $8,000 and the attorney says he gets $2,312 of the total settlement amount. There is not enough left for the medical bills, and the subject obviously gets nothing for pain and suffering since after paying the medical bills he will still owe for medical.
How is this possible? And if the subject has an attorney for a personal injury case, what part does subject play in the settlement, other than signing the check?
